New to Deer Creek Student Enrollment

Welcome to Deer Creek! Our school district offers a convenient way for you to enroll your child through our Online Registration (OLR) Portal.

All families new to our district must complete an application and submit required forms and documentation for children to be eligible to attend school.

Complete a NEW student application if you meet one or more of the following criteria:

  • You have never had a child attend Deer Creek Schools

  • You are an existing Deer Creek family but have a NEW student to enroll
  • Your student has not attended a Deer Creek school for at least one, full school year

Steps To Enroll Your Student:

Step 1

Please gather the required forms:
  1. State law requires that a child’s residence is the school district in which the parent/legal guardian holds legal residence. There can be penalties for falsifying residency. As a result, we need two (2) proofs of residency for enrollment. One from each category below:

    1. A current lease agreement, contract on a home (where you will be residing before the end of the first term enrolled), current mortgage statement, warranty deed, or settlement statement.

    2. Current utility bill (gas, electric or water); Cutoff notices, telephone and cable bills are not accepted.  Bills must be within 30 days of school start date.

  2. Legal Birth Certificate
    • Must be the certificate issued by the state or country where the child was born – not the hospital record.
    • A child must be four (4) years old on or before September 1 to enter PreK, five (5) years old on or before September 1 to enter kindergarten, and six (6) years old on or before September 1 to enter first grade.

  3. Parent/Guardian Driver's License/Photo ID:
    • The biological, foster or adoptive parent may enroll the student.  A driver’s license or government-accepted photo ID is acceptable.
      • Legal Documentation (Adoption/Name Change/Custody): Guardians must have the most recent copies of fully-executed court orders. A student's school record is established using the legal name according to their birth certificate.  Court authorization must be provided to use a different name. In cases of divorce, current custody order documentation is required.

  4. Official Transcript/Report Card:
    • Transcript (grades 9-12) or final/most recent report card (grades 1 - 8) AND withdrawal grades if enrolling during a current school year.

  5. Up-to-Date Immunization Record:
    • The state requires acceptable evidence of adequate immunization or a completed waiver to exempt your child in order to be enrolled in an Oklahoma school.
    • Click HERE for the exemption waiver.
You will need all of these forms with you to complete the application.
